Don't forget to send an out of hours handover form:

for those patients whose deaths are imminent and expected |
|
to provide specific information that will aid the decision to visit and/or how to manage OOH |
|
satisfy the OOH GP that death is expected and thus permit the safe removal to the undertakers and not the public mortuary |
|
ensure patient wishes (eg advanced directives) are shared appropriately |
|
support GPs working OOHs by providing details of other professionals involved |
