Threatened miscarriage – the woman should be at least 7 weeks gestation and have a positive pregnancy test

Suspected ectopic pregnancy - can be seen at any gestation. If patient unsuitable to wait until the next appointment then acute admission to Gynaecology

●A referral letter will be required stating LMP and symptoms. ●Alternatively if the maternity notes are available the need for referral and symptoms can be recorded in those notes and the woman asked to take the notes to the appointment
●They do not see women to provide reassurance scans only - must have symptoms of a threatened miscarriage or ectopic pregnancy
●Sexing of foetus will not happen at EPAU

EARLY PREGNANCY ASSESSMENT UNIT GUIDELINES

Where possible an appointment will be given for the following day but on occasions it may be 48 to 72 hours for the next available appointment.

bulletThreatened miscarriage – the woman should be at least 7 weeks gestation and have a positive pregnancy test. A scan performed earlier than 7 weeks is inconclusive and therefore of no benefit to the woman and it takes scan time unnecessarily, thereby reducing availability of appointments in the unit.
bulletSuspected ectopic pregnancy – a woman with suspected ectopic pregnancy can be seen at any gestation. If the referring doctor is unsure of a patient’s suitability to wait until the next appointment the patient should be referred as an acute admission to Gynaecology.
bullet

All women must be seen by a Midwife, GP or Nurse Practitioner and their condition/symptoms assessed for their suitability to remain at home, before an appointment for EPAU can be booked.

Starving instructions

Please inform the woman that she must not eat after midnight the night before the appointment but can continue to drink until 7am the day of the appointment. After 7am she must not drink anything. This is in case of the need of an operation later in the day.

Please note:

We do not see women to provide reassurance scans, only if they have symptoms of a threatened miscarriage or ectopic pregnancy. If you have concerns regarding a woman’s previous obstetric history, an early referral to Obstetrics is appropriate.

Sexing of the fetus will not take place at the EPAU scan.

Women with hyperemesis should be managed according to the specific guideline and if needing referral should be referred to Maternity.

TERMINATION OF PREGNANCY

I thought other GPs might find this useful: a patient of ours has just had a TOP at BPAS in Leeds, done without a GA. It was done at their premises in the Headrow, and she was given gas and air. This may be useful for women who are unable to tell anyone they are having a TOP, so can't be accompanied home, so can't have GA.

As far as I know all the TOPs at Bfd hospitals are done under GA. Before finding out about this patient I thought that all the women we sent to BPAS had to go to Doncaster to have a TOP under GA                                  Comment from a North Bradford GP
