Diabetes
| All referrals go to Horton Park stating a preference for which clinic and then referrals are passed on from there. |

Drug misuse
| Refer to Eccleshill clinic |
| Use standard referral form |
| Urine test for drug screening prior to ref if possible |
ENT
| All referrals to go to ENT, not named consultant |
| Can direct book into locality clinics or refer to hospital |
| New service started at Eccleshill |
| All thyroid lumps should be referred to ENT |
| Head/neck lumps should go to Mr Ben or Mr Sood |
Mental Health
| Letter or fax to primary care mental health team at New Mill. Can also give referrals direct to Carol. |
| Use common referral form and remote booking will be an option later |
| For locality psychiatry clinic refer to Somerset House |
| Drop in clinic available for psychiatric assessment – book via reception at Somerset House and fax referral letter |
Musculoskeletal
| All referrals for orthopaedics and rheumatology to go through triage |
| Need to distinguish between rheumatology and orthopaedics |
| Need New Zealand scoring for major joint replacement (can be done by physio, doctor or nurse) |
| First port of call should be physio in house (Eccleshill) |
Neurology
| Referral letter to Windhill Green for triage |
| Can now book CT scan direct on normal x-ray card, use ISTC |
| Referrals for epilepsy should go to the epilepsy project at Windhill Green |
Ophthalmology
| SOAP |
| Can remote book but note sometimes quicker to refer to hospital |
Paediatrics
| Refer to consultant who will re-direct to GpwSI |
Pain Management
| Note Dr Cole’s clinic now based at Horton Park. |
| Can refer using chronic pain rehab programme referral forms. |
Physio
| Our service is now located at Eccleshill hospital |
| Referrals should go in the Out tray |
Urology
| All referrals to go to urology triage office. |
| Should use remote booking. |
| Can use either haematuria pathway or fast track pathway for suspected cancers. |
| Patients with haematuria should have MSU and urine cytology done and a direct-booked cystoscopy within 2 weeks |
Vascular
| Venous ulcer clinics available at Wrose, Idle and Shipley |
