►2 or more chronic disease diagnosis
►Over 75years
►4 or more lots of medication lasting more than 6 months
 
PRIMARY OBJECTIVES OF THE PROJECT
To reduce un planned hospital admission to A&E
To reduce unplanned admission to hospital.
To ensure patients get the right care, by the right person, in the right place at the right time.
 
SECONDARY OBJECTIVES OF THE PROJECT
To ensure out of hour services are used appropriately by complex patients having planned and anticipatory care.
To reduce inappropriate “reactive” home visits GP requests
